PAFCAF Annual Awards Nomination Form

Award Categories:

Distinguished Service Award: Do you know someone who has worked in the Child Care for a long time and deserves to be recognized for his or her service contributions? The Distinguished Service Award honors someone who exemplifies excellent work and dedication in the Child Care field.

One Person Can Make a Difference Award: One person really can make a difference, and those people deserve to be honored! Nominate someone today who has made an impact in Child Care that you believe needs to be awarded.

Be the Anchor Award:
In every workplace there is someone who consistently goes above and beyond for the organization and their team. They come in early, work late, and take on responsibilities that are not always part of their job description. This individual often works behind the scenes and may not always receive the recognition they deserve for their passion and commitment. This year, PAFCAF and the annual conference planning committee want to recognize one of these deserving individuals. You can nominate your organization’s “anchor” for this award by following the instructions below.

Nomination Process

The following process for receiving nominations is established:

  • Nominations must be made by members in good standing with PAFCAF.
  • Nominations may be made by any member organization on its official letterhead.
  • Nominations must be complete and submitted via this webform by December 31, 2017.
  • The PAFCAF Board of Directors will select the winner(s), based on the written submissions. Samples of any materials (if relevant) such as program announcements, agendas, reference materials may be submitted.
  • The winner(s) will be announced at the annual conference!

Criteria for Nominations

Nominees must meet the following criteria, and narrative in the nomination submission must support how the nominee exemplifies three or more of the following:

  • The nominee has exhibited meritorious service to his/her organization, community, or state on behalf of children and families in care.
  • The nominee’s performance exceeds the limits of their job description.
  • Peers hold the nominee in high esteem.
  • The nominee has exhibited sustained, extraordinary achievement of assigned tasks.
  • The nominee has overcome obstacles to performance or to self-improvement.
  • The nominee has exhibited leadership where there was no existing leadership.
  • Letter(s) of support from participants or others who are aware of the project/activity and its impact may be submitted and are encouraged.
  • The nominee made special contributions to policy development, technical improvements, business strategy innovations, communication strategies, etc. Submissions in this category must show the individual or organization's substantive contribution to the innovation.

Submissions for these awards will be evaluated by determining the scope of the impact on children and families or on the child welfare system. Submissions are also evaluated on the unique aspects and/or contributions of a particular project or person.

Thank YOU for your nominations! We look forward to honoring these deserving nominees!
