Searching for an experienced master level clinician to develop individualized transition plans and provide in-home interventions for primary caregivers of children residing in our PRTF program.
Works closely with PRTF Therapist and Group Home Care Coordinator to provide transition services for children completing our residential programs. Completes safety assessment of primary caregiver's living environment. Provides on going in-home visitation prior to and post discharge; uses a variety of evidence-based interventions and clinical strategies when working with primary caregiver and child. Creates plan to avoid placement disruption and for managing behavioral issues and crisis situations that may occur. Works to increase parental capacity. Works with family to set up post discharge medical, educational, social support, mental health and other support services for child. Develops interventions to assist the family to develop relationships with naturally occurring community networks. Works closely with in-house multidisciplinary team. to ensure continuity of care. Provides ADL and ILS assessments and training to child as needed.
